  1. Follows through on physician’s orders in accordance with Tabitha’s policies and procedures. If appropriate, receives and records physician’s orders.
  2. Provides skilled observation and/or care to Seniors/clients requiring intermittent professional nursing services. Prepare and administer medication and/or treatments and other nursing services under the direction of the primary RN.  Observes Senior/client, watching for any sign of adverse reaction to medications.
  3. Assesses assigned Seniors/clients in regard to physical, mental and psychosocial status, reporting condition changes or abnormal findings in a timely manner to the primary RN managing the Senior’s/client’s care or immediate supervisor and provides follow-up with family and physician as directed. Identifies teaching needs for Seniors/clients or family.
  4. Takes and accurately records vital signs.
  5. Completes procedures and documents the appropriate information on the Senior’s/client’s medical record in a timely manner according to state/federal regulations and Tabitha’s policies and procedures. Directs particular attention to accuracy and timeliness and follows department protocol for submission of completed documentation.
  6. Adheres to the client’s Home Health Care Bill of Rights.
  7. Coordinates the Senior’s/client’s care and ensures the appropriate treatment is obtained.
  8. Provides reports to primary nurse and other staff involved in care. Uses judgement to determine when concerns should be elevated or communicated urgently based on changes in Senior/client physical or emotional status.
  9. Works on-call, holiday, and weekend shifts on a rotating basis. Also works additional shifts or extended shifts when necessary.    
  10. Adheres to and has knowledge of HHC policies and procedures.
  11. Prioritizes assignments to complete care for Seniors/clients in allotted time, making adjustments throughout the shift to meet changing Senior/client/caregiver/TEAMember/manager needs. 

AGE REQUIREMENT:  At least age 18.


EDUCATION:  Successful completion of a Licensed Practical Nurse course of study.  High School Diploma or equivalent preferred.


EXPERIENCE:  6 month’s experience as an LPN, preferred.  Home Health experience preferred. 


CREDENTIALS:  LPN License in good standing as recorded on the Nebraska State Registry or other Nurse Compact state required.  CPR required.  License must be kept up to date at the expense of the employee.  CPR Certification must be kept up to date at the expense of the employee.  Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required.  Must possess auto insurance at or above Nebraska statutory requirements.
