POSITION SUMMARY:                    

The Clinical Liaison for Tabitha Hospice – A Collaboration with Immanuel is responsible for working with hospitals, physician’s offices, skilled facilities, assisted livings and other community organizations to facilitate the referral process for patients in need of Hospice care. The Clinical Liaison is responsible for reviewing potential hospice referrals, contacting each potential patient/family, discussing their hospice needs, completing all required documentation through the referral system, completing the referral and facilitating the initiation of the referral to the program for admission evaluation/completion. Works as a liaison between the referral source, patient and hospice team to assure a successful admission to services. ESSENTIAL FUNCTIONS:

  1. Works with patients, family members and referral sources to assure all referrals for Hospice are completed according to Tabitha policy.
  2. Coordinates referrals to Tabitha Hospice from the referral source, assuring all information needed at time of referral is provided to the hospice team.
  3. Coordinates referral procedures to ensure necessary information needed for start of care are secured and that there is a smooth discharge plan for those patients transitioning home from the hospital or facility.
  4. Evaluates the patient’s need for hospice care through gathering pertinent medical information, overall patient history and patient assessment.
  5. Evaluates patient’s medical information to determine Medicare, Medicaid, and private insurance reimbursement, and acts on any potential concerns as identified.
  6. Provides information to patients regarding financial considerations, including general information regarding the Medicare Hospice Benefit, Medicaid, private insurance, etc.
  7. Advises, counsels, teaches, and supports patients and/or family members in assuming responsibility/advocacy for personal health care needs, decision making, and ongoing planning and services coordination.
  8. Networks with community agencies and representatives of insurance companies and/or funding sources as appropriate.
  9. Documents and maintains accurate patient records related to client referrals, admissions, service planning, and discharges.
  10. Accesses, inputs, and extracts information from medical record systems in facilitate the referral and intake process.

AGE REQUIREMENT: At least age 18.

 

EDUCATION:  Successful completion of a Registered Nurse course of study.  Bachelor’s degree in Nursing preferred

 

EXPERIENCE: Two years of acute care/long term care nursing experience within the last five years preferred. Experience in community health and/or hospice preferred. Medicare and Medicaid training/experience preferred.  Experience and/or training in computerized information systems preferred.
