Passionate about Hospice and leading a team? The Monarch by Eastmont, is looking for you to fill a vital need. We are currently looking for our next Hospice Coordinator to manage the day-to-day cares and operations of our 6-bed hospice home.
Our Hospice Coordinator:
- Provides referral management with hospice care agencies, hospitals, and the community. Provide compassion, guidance and support to residents and their families, answering questions on any/all concerns in regard to hospice care and review clinical care needs.
- Provide leadership support in the weekend on call rotation as part of the Clinical Leadership team.
Salary: Starting at $55,000
**Compensation will be based off licensure status and years of experience
