Assisted Living & Memory Care Services
In assisted living and memory care communities, CHP provides consistent on-site medical coverage through nurse practitioners and physician assistants, supported by in-office triage nurses who assist with chronic care management, clinical coordination, and day-to-day facility support. In select communities, CHP also offers on-site phlebotomy services for routine laboratory draws, helping streamline care and reduce disruption for residents and staff.
Skilled Nursing & Post-Acute Services
In skilled nursing and post-acute settings, CHP delivers advanced practice provider coverage and Medical Director Services, supporting higher-acuity residents, post-hospital transitions, and complex medical management. Our teams work closely with facility leadership to promote continuity of care, regulatory compliance, and coordinated transitions across levels of care.
